Make a referral

At Rethink Advocacy, our goal is to ensure our services are accessible, relevant, and inclusive to everyone, regardless of their background or identity. ​

To do this effectively, we collect demographic information, such as data on protected characteristics, to help us see who we are reaching and where we need to do more to engage with underrepresented groups. ​

Please provide us with the D&I information on the referral when you can. This is vital information that can help us improve our service, as well as provide the best advocacy support to each individual.

If you are completing a self referral, this section is optional and you do not have to give us information you are not comfortable sharing.

This section is for you to tell us what you need an advocate for. We work as an issue based service which means we need to know what specific problem you are having and what you need the advocate to help you with. 

If you have multiple things you would like an advocate to help you with, detail them but bear in mind the advocate may only be able to work on one at a time. 

For professionals filling out a referral for, we need seprate referral forms for each advocacy remit you are referring for. For example one for IMCA serious medical treatment and one for Care Act safeguarding. 

Too few details can mean that we follow up on what the advocacy issue is, alternatively a large chunk of information may also result in us clarifying what work is needed.

If they have capacity have they consented to the referral. If they have not consented we cannot accept

This section is only needed for Independent Mental Capacity Advocacy (IMCA) referrals. You can leave blank if you are not making this type of referral. 

If you are making an IMCA referral this section had to detail the person who is making the decision that the referral is for. For example the social worker who is making the change of accomodation decision. 

If you are a professional making a referral for a client who lacks capacity, this section is to detail when a mental capacity assessment was completed. 

This is especially important for Independent Mental Capacity Advocacy (IMCA) referrals.
